At 4:50pm on Friday 9th October 2015 Emergency services were called to Gladstone Road, Levin.
There were reports of a person falling off a horse.
Unfortunately, despite the efforts of emergency services, a 39 year old female from Levin died at the scene.
The name of the deceased will be released after her family have been notified.
The death has been reported to the coroner.
