Statutory Declaration in respect of an alleged vehicle offence made under the Oaths and Declarations Act 1957
Complete this form if you wish to transfer the liability of a moving vehicle (red light / safety camera) infringement notice to another driver.
This Statutory Declaration is made in the event someone else using your vehicle when it was allegedly involved in a moving vehicle offence.
Ensure you complete all applicable sections and have the form witnessed by a Justice of the Peace, Solicitor, Registrar/ Deputy Registrar of the Court, an Authorised Member of Police that is ranked Sergeant or above, or any other person authorised by the Minister of Justice to take a statutory declaration.
The declaration is made under the Oaths and Declarations Act 1957 and is subject to the conditions under the Land Transport Act 1998 if a person makes a false declaration.
Please note: You may only use this form to transfer liability for safety camera notices that were issued by New Zealand Police. If you wish to transfer liability for safety camera notices issued by another Issuing Authority (e.g. New Zealand Transport Agency) then you must contact that Issuing Authority.
