Identity Referee Statement form
When an individual makes a request to Police for their personal information under the Privacy Act 2020, Police are required to be satisfied as to the requester’s identify before providing access to the information.
If you are able to submit to Police a clear and readable photograph of yourself holding your evidence of identity, an identity referee’s statement is not required.
If you are not able to provide a clear and readable photograph of yourself holding your evidence of identity, you must submit an identity referee’s statement to confirm that the submitted evidence of identity is yours.
This form confirms that the referee has sighted your evidence of identity, compared the photographic image with you, and confirms you appear to be the same person.
